Important Notice to Former HSA Members

The UTU has been informed that Health Services Association (HSA) has been placed in liquidation by the Minnesota Department of Commerce.

If you were insured by HSA you may be eligible for replacement coverage under Group Policy GA-23111 issued by United HealthCare Insurance Company. GA-23111 is sponsored by the railway labor organizations who represent railroad employees.

You and your dependents are eligible to enroll under GA-23111 if you worked for a railroad in a position represented by one of the railway labor organizations while you were in active service.

You are not eligible to enroll if:

• you worked in a non-agreement position (you did not work under a collective bargaining agreement), or

• you are currently working, and had HSA coverage that supplements your active coverage under The Railroad Employees National Health and Welfare Plan.

If you are eligible to enroll, you may obtain this coverage retroactive to May 1, 1999, provided your enrollment is mailed on or before June 15,1999. You may actually enroll any time on or before July 31,1999, but if you enroll after June 15,1999, your GA-23111 coverage will be effective the first of the month following the month of your enrollment.

GA-23111 offers the following plans of insurance:

• Plans D or F for individuals eligible for Medicare (Note: Plan F is only available effective June 1,1999. If you select coverage under Plan F, you will be covered under Plan D in May, 1999, and Plan F beginning June, 1999. Plans D and F are identical except that Plan D covers prescription drugs, but Plan F does not cover prescription drugs.

• Plan E for individuals covered under The Railroad Employees National Early Retirement Major Medical Benefit Plan;

• Plan C for all other individuals.
