Significant Network Change - Atlanta Network Physician Group Termination - Premier Medical Group Effective October 21, 1999

Dear Joe and Stu:

The following information is regarding a change in the Aetna U.S. Healthcare provider network currently utilized by your employees.

What's Happened?

Premier Medical Group located in Roswell, Georgia terminated from the Atlanta, Georgia network for all products. This group is comprised of four primary care physicians (PCPs) and has announced that it will shortly be closing its doors to the public. The address for the provider group is as follows:

Provider Access

Access to providers will not be impacted by this termination. Members may access care from two alternate participating provider groups in the area. These provider groups are within 15 miles or less than the closing group's location. The addresses are as follows:

Member Disruption

There are 77 Railroad employees/dependents affected by the change and will be required to select a new PCP.
